Record Closure Request

Request Closure of Your Identity Record

To Close or Deactivate a KYC, KYB, or UBO Record, Please Complete the Official Disclosure Form, sign it, and Submit it by Email. Our Data Protection Officer will Review and Process it.

Important Manual Submission Required

Record closure cannot be done online. You must download the form, fill it in, sign it, and email it back to us. Requests without a signed disclosure form cannot be processed.

How to Submit Your Closure Request

Follow these Three Simple Steps

    Download the Disclosure Form

    Click the button below to download the official Closure & Disclosure Form. This form is required for every closure.

  2. 2
    Fill and Sign the Form

    Complete all sections of the form. Include your full names, identifier (KYC, KYB or UBO Reference), reason for closure, and sign the Declaration.

  3. 3
    Email the Signed Form

    Send the signed and completed form to our official DPO email address. A confirmation of receipt will be sent to you personal or business email shortly.

Data Protection Notice

All closure and disclosure requests are processed in accordance with Law N° 058/2021 of 13/10/2021 relating to the protection of personal data and privacy in Rwanda. Your request and supporting documents will only be used for identity verification and compliance purposes.

Once your signed form is received, our Data Protection Officer will review the request and may contact you for additional verification. Approved closures are logged in the audit trail for compliance. You may also exercise your rights of access, rectification, or objection by contacting dpo@lfsg.co.rw.
